Best password vault?

I have a lot of web site accounts. I mean a lot.

I've memorized passwords tied to money, but I'm looking for some sort of password vault for less important accounts (e.g. Kongregate, Tetris Friends, other forums, etc.).

Any suggestions?

I just make all my passwords the same one. Usually "12345" or just "password". Life is simple.

I read an informative article on password management. I was looking into cross-platform solutions - I have an android phone, tablet and two computers. I found it helpful.

Fine until your "insert favorite online game" account is hacked. I've been hacked in my Blizzard accounts with way more difficult passwords than that.
